BCLTCC Home & Community Services Committee Meeting Notes- Seniors Helping Seniors of Bucks County 2:30 – 4pm 1/30/14

 Present: Dana & Devon Ainsworth (A Way to Stay Home Care), Lisa Nekoranik (Bayada),  Christine Schonewolf (BrightStar), Sarah Floyd (Carpenter Law),  Beth Ann Shewbrooks &  Cathy Flynn (Comfort Keepers), Bob Parvin, Nancy Feather, Heidi Brush and Jessica Vinciguerra (Crestview – Genesis), Mary Ann DuGan (Friends Home and Village), Donna Manning (Grandview Hospital), Beth Hohberger- McCarty (Hospice Compassus), Catherine Warshaw (Innovations Home Care), Erin Williams (LIFE St. Mary),  Sean Lynch (Living Care Home Services), Pat Harris (Lower Bucks Hospital), Chris Campbell & Jerry Lawson (Right at Home),  Bryan Adler (Rothkoff Group)  Barbara Marte (Seniors Helping Seniors),  Laura Poust (SMMC ), Anda Durso (Sunrise Senior Living Lower Makefield)

 Agenda

  1. Welcome and Introductions
    Erin Williams opened the meeting, and attendees introduced themselves. Dana Ainsworth is now co-chairing this meeting.
  2. Review of Minutes
    Minutes from the last meeting were sent in October and are on the BCLTCC website.
  3.  Sherry Marcantonio, SVP Health Quality Partners (HQP)
    Ms. Marcantonio gave a presentation about the community-based model of advanced care HQP developed. The program redefines being old and chronically ill as a condition requiring professional medical management. According to independent analysis by Mathematica, HQP has reduced hospitalizations and cut Medicare costs significantly.  Read what THE WASHINGTON POST said about the program by clicking here.  Sherry also recommended looking at videos on HQP website click here.  She recommended an online article by Donald M.     October 23, 2014 – Location to be determine
  6. Schedule next Home and Community Services Committee Meeting:
    The next meeting will be in early April, 2014; the exact date, location and topic to be determined. Please send your thoughts to Erin!
