- Welcome and Introductions
Erin Williams opened the meeting, and attendees introduced themselves. Dana Ainsworth is now co-chairing this meeting.
- Review of Minutes
Minutes from the last meeting were sent in October and are on the BCLTCC website.
- Sherry Marcantonio, SVP Health Quality Partners (HQP)
Ms. Marcantonio gave a presentation about the community-based model of advanced care HQP developed. The program redefines being old and chronically ill as a condition requiring professional medical management. According to independent analysis by Mathematica, HQP has reduced hospitalizations and cut Medicare costs significantly. Read what THE WASHINGTON POST said about the program by clicking here.
