The Bucks County Long Term Care Consortium Hosts a Presentation by Health Quality Partners
Sherry Marcantonio MSW, ACSW, SVP of Health Quality Partners (HQP) will speak about the community-based model of advanced care they have developed. The program redefines being old and chronically ill as a condition requiring professional medical management. According to independent analysis by Mathematica, HQP has reduced hospitalizations by 33% and cut Medicare costs by 22%.
